New patients: Please complete your intake forms prior to scheduling your visit. Once we receive them, we will contact you to schedule and accept your deposit. Dr. T reviews every patient's records before their visit, and your completed forms are part of that preparation.
Existing patients: If your information has changed, please submit updated paperwork at least 48 hours before your next appointment.
